📢 Holiday Closure Notice
In observance of Juneteenth, all Tandem Health medical sites, pharmacies, administrative offices, and support services will be closed on Friday, June 19.
Juneteenth commemorates the end of slavery in the United States and serves as a time to reflect on our nation's history, celebrate freedom, and honor the ongoing pursuit of equality for all.
We will resume normal business hours on Monday, June 22.
We wish our patients, staff, and community a safe and meaningful Juneteenth.

In this role, you'll work alongside our clinical teams to improve patient outcomes by supporting preventive care, chronic disease management, patient outreach, care gap closure, and quality improvement initiatives. This position plays a vital role in helping ensure our patients receive timely, high-quality care while advancing Tandem Health's mission of improving community health.

Yesterday was a powerful reminder of what happens when women come together with authenticity, purpose, and a willingness to lift one another up.
