There come times in your life that you never plan for. The feeling of loss of someone special from the world is one of them. I just learned of the passing of a man that had a huge impact on my life, especially in the world of EMS. Raj, I don’t even know how to start to process your loss. I will be the first to admit that in the last few years we did not have much contact. Though I thought of you often I never would reach out, even just to say “Hey, what’s up?”. With the hustle and bustle of life I had missed calls from you that I wish I had taken the time to return.

Raj was there for me in EMS from the start. He was abtrue example of the compassionate Paramedic and taught me a lot. We spent hours just talking in the EMS Supervisor office, at the hospital, or in a parking lot window to window. We talked about the job, our families, told awesome stories and just had a great time. You helped make me the provider that I am today. Yoir support for the Squad Truck team and ALL EMS providers you met over the years will never be forgotten.

I will most defiantly never forget the time you called and asked if we could use my brand “new to me” truck for an errand. When I ssid sure, what do you need, you emphatically yet jovially replied, “I need you to help me pick up a goat”. I honestly thought you were joking, but the goat pellets left in the back of my truck ensured it was infact for real.

Raj you will be forever missed, but NEVER forgotten. I love you brother.

Good Information!!

If you work in an ED you surely know what “5&2” means. It’s the combination of Haldoperidol and Ativan that is given to control agitated patients.

In EMS we have switched to Ketamine with great success even though the drug has recently been maligned by the press without evidence to substantiate their claims.

Would Ketamine work in an ED compared to Haldol and Midazolam? A recent study asked this exact question.

*Patients in the ketamine group were treated with a 5 mg/kg IM injection (split into multiple syringes; 50 mg/mL concentration, max of 4 mL per IM site).
*Patients in the haloperidol/midazolam group were treated with a single IM injection consisting of 5 mg midazolam and 5 mg haloperidol.

What were the results?
1. A total of 308 patients were enrolled in the study.
2. The median time to sedation was 14.7 minutes for haloperidol/midazolam versus 5.8 minutes for ketamine (difference 8.8 minutes [95% confidence interval (CI) 3.0 to 14.5])
3. Five patients in the ketamine arm and 2 patients in the haloperidol/midazolam arm experienced serious adverse events (difference 7.5% [95% CI -4.8% to 19.8%]).

Six patients (15%) in the Haldol/Midaz group and 5 patients (12%) in the Ketamine group needed a rescue medication (defined as benzodiazepines, antipsychotics, or other sedative medications, and administered at the attending emergency physician’s discretion.

Serious adverse events were as follows:
1. Apnea treated with BVM: Haldol/Midaz (1); Ketamine (2)
2. Supplemental O2: Haldol/Midaz (1); Ketamine (1)
3. Laryngospasm treated with jaw thrust and airway repositioning: Haldol/Midaz (0); Ketamine (1)
4. Dystonia: Haldol/Midaz (0); Ketamine (1).

No patients required endotracheal intubation or ICU admission.

Handtevy comments: Ketamine led to a significantly faster time to sedation and did not lead to patient harm. It has proven itself once again to be an excellent option for these situations.
