This waiver is in addition to the one you may already have on file.
The new Sensory Room requires a liability waiver, and anyone under 18 must have one on file. You can save time by filling it out through the link provided or complete it when you arrive before entering the sensory area.
Be sure to complete your smart waiver ahead of time! Parental Responsibility: Our sensory room is always parent-directed. You are responsible for your child and must always be with them while in the sensory room.
