You will be notified of this and closer to the time, you will receive a notification. You can then log in and complete a simple follow-up guided assessment tool to help your doctor decide how well controlled your chronic illness is.If you make use of any remote monitoring devices e.g. glucose monitors, you can share any information which will give your doctor a more detailed impression of how well controlled you are.
