The International Agency for Research on Cancer, part of the World Health Organization, has classified combined estrogen-progestogen oral contraceptives as Group 1, carcinogenic to humans, since 2005. This classification was detailed in Monograph Volume 91 and reaffirmed in Volume 100A in 2012. It remains unchanged as of the most recent reviews.

Group 1 designation indicates sufficient evidence from human studies that the agent can cause cancer. The category includes well-known hazards such as to***co smoke, alcohol consumption, processed meat, asbestos and ultraviolet radiation. It identifies the presence of a carcinogenic hazard rather than the magnitude of risk in everyday use or the overall health impact.

Evidence from large-scale studies shows that current or recent use of combined oral contraceptives is associated with a modest increase in breast cancer risk, typically in the range of 20 to 24 percent relative risk. The absolute increase remains small, especially among younger women, where baseline rates are low. This elevated risk diminishes over time and generally returns to the level of never-users within approximately ten years after discontinuation.
